The grant period for the Senior SNAP Enrollment Initiative ended yesterday, April 30, 2018. First, I’d like to think you all for being such great partners throughout the initiative and your hard work and dedication to helping seniors enroll in SNAP. As we wrap up the grant period there are a few important final reports and a call that must be completed to receive the final grant payment. As a reminder, on time reporting, and participation in the monthly check-in calls are requirements for payment of the grant.

 

1.            The final monthly report can be found at https://ncoa.tfaforms.net/313929. This report is due by COB Tuesday, May 15, 2018.

2.            The final financial report is also due by COB Tuesday, May 15, 2018. Please use the form attached to this email to complete this report. Please return this report to Lyndsea Seril, lyndsea.seril@ncoa.org, and copy me on this email. For reference, I have copied Lyndsea on this email.

3.            This month, on Wednesday, May 16, 2018 at 3pm EST, we will have the final monthly check-in call. During this call we will have discuss the performance of your initiatives over the course of the grant year. Please be prepared to talk about: whether or not you met your goals, the activities that led to your success, any obstacles you encountered and how you responded, the impact your project has had in your community, and your plans for sustainability. Please let me know if you have any questions.
