Changes to reporting form:

Outputs and Outcomes section:

Previously there were several placeholder questions as the client follow-up survey was being finalized. There are still questions asking you to report on client responses regarding enrollment status from the follow-up survey, and those questions have been updated to include guidance on where to find those numbers in the Reporting page of the survey workbook (table “Reporting on Enrollments”). Some other placeholder questions have been removed, but that doesn’t mean they are not important or will not be asked later. Rather, we don’t want to require monthly reporting of information we don’t need or plan to review monthly.
There is now a field to allow you to upload the latest version of your survey workbook. For this month, the field is optional, but if you are experiencing any challenges with the survey (e.g., many clients quitting the survey before the end) or the workbook itself, sharing the workbook is encouraged as it would allow us to have a more detailed conversation with you about those challenges. If you elect to upload the workbook, please remember to remove any PII (personally identifiable information, such as client names, contact information, SSN, etc.) from the version shared.

Narrative section: As requested by a participant on the TA call, in the Narrative section of the report, we have added an “Optional File Upload” field for any materials you wish to share to complement the narrative. This field is entirely optional.

This field should not be used for client success stories. Please continue to report client success stories at least once per quarter in the Success Stories section.
Please note the upload field can only be used to upload a single file of up to 1GB (slow upload speeds could lower this limit), so if you wish to upload multiple documents, we recommend consolidating files into a single PDF.

The February monthly report is due Monday, 3/17, since the 15^th falls on the weekend.
