In my experience, unless they check the box requesting help with Medicare premiums on their Medicaid application, they won't always be put through for QMB. Which to me is strange. I would think it would be automatic if they qualified for Medicaid. But that's not always the case here in MO.

These limits are higher than used by many states. Our Medicaid eligibility income limit was increased to 100 percent of FPL in June 2014 when Indiana eliminated the Spend-down provision.

In Indiana QMB is considered a category of Medicaid. Beneficiaries who are eligible for QMB status, therefore, are issued Hoosier Health Insurance (Indiana Medicaid) cards to show to medical providers.

I've never seen a QMB card.  Can anyone tell me about QMB cards?  Or, if I have misinterpreted this note, please set me straight.  I am in Florida, and maybe we don't have QMB cards.
