This is a reminder that reporting for the month of April (the reporting period ending 4/30/2018) is now open. Please submit your report by COB on
Tuesday, May 15 at the following link: https://ncoa.tfaforms.net/workflows/start/2
As a reminder, we are asking you to start reporting aggregate demographic data (e.g., age group, gender, race, health status, etc.) for the May report due June 15, which means that you should be collecting this data for the month of May.
For the April report, demographic data is not required, but please report any data you already have.
If you are unable to complete your report by May 15, I would greatly appreciate it if you would notify me ahead of time at
lauren.popham@ncoa.org.
If you have any staffing changes to your core BEC team, please email Randy Feliciano
randy.feliciano@ncoa.org and Genevieve Waterman
Genevieve.waterman@ncoa.org so that they can update your records.
