I wanted to send around some talking points that you can use when asking your clients for demographic data.

I am going to ask you some demographic questions. You can skip certain questions or not provide this information at all. If you decide not to provide some or all demographic information, it will not affect your eligibility for
benefits.

However, please know that your information is very valuable to us. We use it to learn who is being reached by this program and to improve our services for others like yourself.

We follow very strict rules to protect all of your information and to keep it private. We do not report individual responses to our funder; instead it will be combined with other clients’ data and reported in that way.
As a reminder, when you ask about health and veteran status, use the questions below. I think the other demographic questions including age, income, race/ethnicity, marital
status, disability status are self-explanatory, but please let me know if you have any questions.
Health status: “Would you say your health is excellent, very good, good, fair, or poor?”
Veteran status: “Have you served in the military?”
Thanks for incorporating these questions into your client screening. Better insight into the clients we all serve will help us tailor our services and outreach to better
help older adults and adults with disabilities.
